Please provide company information, or if enrolling as an individual, your personal information on page 1. On page 2, please note the names, dates of birth, phone contact details and the USI of each participant. If enrolling participants in Refresher training, please provide details of previous training (Previous Certificate No).
Each student must have a Unique Student Identifier in order to receive their qualification or statement of attainment. For instructions on how to create USI’s, please visit the Unique Student Identifier page found on our website.
Enrolment of participants takes place once the 2 page enrolment document has been completed and returned, and it is confirmed that places are available in the nominated course.
Each participant is required to complete this form individually. The form must not be completed by a third party or company official and be given to the trainer on the training day before the course commences.

CPR Refresher
Provide cardiopulmonary resuscitation Training Course
HLTAID001
- This course provides participants with the confidence and competence required to perform CPR on an unconscious casualty. This training course is designed for any person whom may need to perform CPR to utilise D-R-S-A-B-C-D during the course of their employment. This training course is taught according to the Australian Resuscitation Council (ARC) guidelines. This training course takes personnel through both the theory of why we do CPR and many practical exercises that emphasise the ‘how’ we perform CPR.
Training Venues:

Course Time:
0800 am to 1200 pm (4 hours)
Assessment:
Participants will be assessed via a questionnaire and practical demonstration of skills. Participants with literacy/numeracy difficulties or physical conditions should speak with the Trainer to discuss alternative assessment approaches. Applicants will be required to undertake physical activity to meet all the assessment criteria to a satisfactory level.
Qualification:
On successful completion, each participant will be issued with a Statement of Attainment.
First Aid- Provide First Aid- HLTAID003
- This course is aimed at anyone who wants to learn essential first aid skills and who may be required to provide a first aid response in a range of situations.
- Participants will attain the practical application and understanding required to provide first aid response to a casualty. Participants will be able to:

- Respond to an emergency situation
- Apply appropriate first aid procedures including:
- performing at least 2 minutes of uninterrupted single rescuer cardiopulmonary resuscitation (CPR) (5 cycles of both compression’s and ventilation’s) on an adult resuscitation mannequin
- performing at least 2 minutes of uninterrupted single rescuer CPR (5 cycles both compression’s and ventilation’s) on an infant resuscitation mannequin
- Responded to at least two simulated first aid scenarios contextualized to the candidate’s workplace/community setting, including:
- conducted a visual and verbal assessment of the casualty
- demonstrated safe manual handling techniques
- post-incident debrief and evaluation
- provide an accurate verbal or written report of the incident
- Communicate details of the incident
- Evaluate the incident and own performance

Qualification:
On successful completion of this course participants will be issued with a National Recognised Statement of Attainment for HLTAID003 Provide First Aid and HLTAID001 Provide Cardiopulmonary Resuscitation along with a wallet sized card. The Unique Student Identifier (USI) must be supplied and verified prior to Certificates being issued.
Your Statement of Attainment does not have an expiry date, however, the Australian Resuscitation Council guidelines state CPR skills should be refreshed every 12 months.
