Please provide company information, or if enrolling as an individual, your personal information on page 1. On page 2, please note the names, dates of birth, phone contact details and the USI of each participant. If enrolling participants in Refresher training, please provide details of previous training (Previous Certificate No).
Each student must have a Unique Student Identifier in order to receive their qualification or statement of attainment. For instructions on how to create USI’s, please visit the Unique Student Identifier page found on our website.
Enrolment of participants takes place once the 2 page enrolment document has been completed and returned, and it is confirmed that places are available in the nominated course.
Each participant is required to complete this form individually. The form must not be completed by a third party or company official and be given to the trainer on the training day before the course commences.

Fire Extinguisher -PUAWER008B- Confine small workplace emergencies
- Our Fire Extinguisher training course will ensure that all of your employees are suitably trained in the safe use of Fire Extinguishers, Fire Hose Reels and Fire Blankets.

Course Time:
1230pm to 1500 pm (2.5 hours)

- Safety procedures when fighting fires
- Firefighting Tactics
- Practical Fire Extinguisher, Fire Blanket & Fire Hose Reel scenarios
Assessment:
Participants will be assessed via a questionnaire and practical demonstration of skills.
Qualification:
On successful completion, each participant will be issued with a Statement of Attainment.
